The Effect of Using a CPM Device in Early Rehabilitation Following Anterior Cruciate Ligament (ACL) Reconstruction for Injured Individuals

Abstract

The current study aimed to identify the effect of using a continuous passive motion device (CPM) within an early rehabilitation program to improve the functional performance of the knee joint (range of motion and pain degree) after anterior cruciate ligament reconstruction surgery (ACL). The researchers used the experimental method with a single-group experimental design featuring pre- and post-measurements. The research sample included (7) injured adult athletes aged between (22-28) years old, who were clients at the Khatawat Al-Hayat Specialized Center in Kirkuk Governorate. The experiment lasted from December 2025 until April 2026. The study tools included measuring the range of motion (flexion and extension) using a goniometer, and a standardized scale for pain degree linked to functional flexion angles. The data were statistically processed using the "Wilcoxon" test for comparisons and to determine the effect size (r).

The results showed statistically significant differences (Sig<0.05) between the pre- and post-measurements in favor of the post-measurement in all investigated variables; as the arithmetic mean of the pain degree decreased from (7.857) to (0.892) degrees, with a strong effect size reaching (0.78). Additionally, the arithmetic mean of the knee flexion angle increased from (〖37.00〗^∘) to (〖135.00〗^∘), with an effect size of (0.77), and the deficit in full extension decreased from (〖34.20〗^∘) to reach (〖0.800〗^∘), with an effect size of (0.78). The study concluded that integrating the (CPM) device contributes decisively and effectively to accelerating the recovery of the range of motion and pain control as a helper factor that prepares the joint for an optimal response to the rehabilitation program. The study recommended the necessity of including the (CPM) device as a core pillar in early rehabilitation protocols during the first weeks following surgery.
